What is sepsis?

Sepsis is a potentially life-threatening medical condition that occurs in response to an infection. It is characterized by a dysregulated immune response to an infection, leading to systemic inflammation and widespread tissue damage throughout the body. Sepsis can progress rapidly and may lead to septic shock, multiple organ failure, and death if not promptly diagnosed and treated.

Sepsis occurs when the body's immune system overreacts to an infection, triggering a cascade of inflammatory responses that can lead to tissue damage, impaired organ function, and systemic complications. Common sites of infection that can lead to sepsis include the lungs (pneumonia), urinary tract (urinary tract infections), abdomen (peritonitis), skin (cellulitis), and bloodstream (bacteremia). However, sepsis can develop from any type of infection, including bacterial, viral, fungal, or parasitic infections.

The signs and symptoms of sepsis can vary depending on the underlying infection, the severity of the inflammatory response, and the individual's overall health status. However, common signs and symptoms of sepsis may include:

Fever: Elevated body temperature (above 38°C or 100.4°F) is a common sign of systemic inflammation and infection. However, some individuals with sepsis may have low body temperature (hypothermia) instead of fever.

Rapid heart rate (tachycardia): An abnormally fast heart rate, often exceeding 90 beats per minute at rest, may occur in response to the body's efforts to maintain blood flow to vital organs in the face of systemic inflammation.

Rapid breathing (tachypnea): Increased respiratory rate, often exceeding 20 breaths per minute at rest, may occur in response to metabolic acidosis (a buildup of acidic substances in the blood) and impaired oxygen delivery to tissues.

Low blood pressure (hypotension): Decreased blood pressure, particularly systolic blood pressure below 90 mmHg, may occur in severe cases of sepsis or septic shock, leading to inadequate tissue perfusion and organ dysfunction.

Altered mental status: Confusion, disorientation, agitation, or decreased level of consciousness may occur in individuals with severe sepsis or septic shock, reflecting impaired brain function due to systemic inflammation and hypoperfusion.

Decreased urine output: Oliguria (decreased urine output) or anuria (absence of urine output) may occur in response to decreased renal blood flow and acute kidney injury (acute renal failure) in severe cases of sepsis.

Hypoxemia: Low oxygen levels in the blood, often detected by pulse oximetry or arterial blood gas analysis, may occur due to impaired gas exchange in the lungs or decreased oxygen delivery to tissues.

Lactic acidosis: Elevated blood lactate levels, often detected by blood tests, may occur in response to tissue hypoxia and anaerobic metabolism in severe cases of sepsis or septic shock.

Sepsis is a medical emergency that requires prompt recognition and intervention to prevent progression to severe sepsis, septic shock, or multiple organ failure. Diagnosis of sepsis is based on clinical assessment, laboratory tests (such as blood cultures, complete blood count, and serum lactate levels), and imaging studies (such as chest X-ray or ultrasound). Treatment of sepsis typically involves supportive care, intravenous antibiotics to target the underlying infection, fluid resuscitation to maintain hemodynamic stability, vasopressor medications to support blood pressure, and other interventions to manage complications and organ dysfunction.

Early recognition and management of sepsis are critical for improving outcomes and reducing mortality. Sepsis protocols and guidelines emphasize the importance of rapid diagnosis, early initiation of appropriate treatment, and ongoing monitoring of clinical response to therapy. Prompt transfer to an intensive care unit (ICU) or specialized critical care setting may be necessary for patients with severe sepsis or septic shock. Overall, a multidisciplinary approach involving healthcare providers, infection control specialists, critical care teams, and other members of the healthcare team is essential for optimizing outcomes in patients with sepsis.
