What is Ebola virus disease?

Ebola virus disease (EVD), formerly known as Ebola hemorrhagic fever, is a severe and often deadly viral illness caused by infection with Ebola virus. The Ebola virus belongs to the Filoviridae family and is named after the Ebola River in the Democratic Republic of the Congo (formerly Zaire), where it was first identified in 1976 during an outbreak.

Ebola virus is transmitted to humans from wild animals and is typically spread through direct contact with the bodily fluids of infected animals, such as fruit bats, primates (apes and monkeys), or through handling or consumption of infected bushmeat. Once introduced into the human population, Ebola virus can spread from person to person through direct contact with the blood, secretions, organs, or bodily fluids of infected individuals, including through broken skin or mucous membranes.

Ebola virus can cause severe illness in humans, with symptoms ranging from fever, fatigue, and muscle pain to more severe manifestations such as vomiting, diarrhea, abdominal pain, and unexplained bleeding (hemorrhage). The severity of illness can vary depending on factors such as the strain of Ebola virus, the individual's immune response, and the presence of underlying health conditions.

The case fatality rate (the proportion of people who die from the disease) in Ebola outbreaks has varied widely, ranging from 25% to 90%, with an average case fatality rate of approximately 50%. Ebola virus disease can lead to multiorgan failure, shock, and death, often within a few days to a few weeks after symptom onset.

Ebola outbreaks occur sporadically in areas of Central and West Africa where the virus is endemic, particularly in remote and rural areas with limited healthcare infrastructure and resources. The largest outbreak of Ebola virus disease occurred in West Africa from 2014 to 2016, primarily affecting Guinea, Liberia, and Sierra Leone. The outbreak resulted in over 28,000 cases and more than 11,000 deaths, highlighting the devastating impact of Ebola virus disease on affected communities and the importance of rapid and effective outbreak response efforts.

Preventive measures to reduce the risk of Ebola virus transmission include:

Avoiding contact with wild animals: Avoid handling or consuming bushmeat and avoid contact with fruit bats, primates, or other animals that may carry Ebola virus.


Practicing good hygiene: Wash hands frequently with soap and water or use an alcohol-based hand sanitizer, especially after caring for sick individuals or handling potentially contaminated materials.


Implementing infection control measures: Healthcare workers and caregivers should follow strict infection control practices, including wearing personal protective equipment (such as gloves, masks, gowns, and eye protection) and safely handling and disposing of contaminated materials.


Promoting safe burial practices: Ensure safe and dignified burial practices for individuals who have died from Ebola virus disease to prevent further transmission of the virus.


Surveillance and early detection: Implement surveillance systems to detect and respond to suspected cases of Ebola virus disease promptly, including case identification, contact tracing, and laboratory testing for confirmation of diagnosis.


Vaccination: In recent years, a safe and effective vaccine against Ebola virus has been developed and used in outbreak settings to protect healthcare workers and at-risk populations from infection.

Effective outbreak response efforts require a coordinated and multisectoral approach involving healthcare providers, public health authorities, community leaders, and international organizations to control the spread of Ebola virus disease, provide medical care to affected individuals, and address the underlying social, economic, and environmental factors contributing to outbreaks.
