In some cases, combination antibiotic therapy may be necessary, particularly in patients with severe or complicated infections.

Fluid resuscitation: Intravenous fluid administration is crucial for restoring intravascular volume and improving tissue perfusion in patients with sepsis-induced hypotension or shock. Fluid resuscitation helps to maintain hemodynamic stability, prevent organ dysfunction, and support vital organ function. Balanced crystalloid solutions (such as normal saline or lactated Ringer's solution) are commonly used for fluid resuscitation, although the choice of fluids may vary based on the patient's clinical condition and fluid status.

Vasopressor therapy: In patients with septic shock or refractory hypotension despite fluid resuscitation, vasopressor medications may be used to support blood pressure and improve tissue perfusion. Vasopressors (such as norepinephrine, dopamine, or vasopressin) help to constrict blood vessels and increase systemic vascular resistance, thereby raising blood pressure and maintaining organ perfusion. Vasopressor therapy is typically initiated in the intensive care unit (ICU) under continuous hemodynamic monitoring.

Mechanical ventilation: In patients with sepsis-associated respiratory failure or acute respiratory distress syndrome (ARDS), mechanical ventilation may be necessary to support oxygenation and ventilation. Close monitoring of vital signs, laboratory parameters, and clinical response to treatment is essential for guiding ongoing management and adjusting interventions as needed.

Source control: Prompt identification and treatment of the source of infection are essential for preventing ongoing microbial replication and systemic spread of infection. Source control measures may include surgical drainage or debridement of infected wounds, removal of infected devices (such as intravenous catheters or prosthetic implants), and other interventions to eliminate the source of infection.

Adjuvant therapies: Additional therapies such as corticosteroids, intravenous immunoglobulin (IVIG), activated protein C (if available), and other immunomodulatory agents may be considered in selected cases of sepsis, although their use remains controversial and is not routinely recommended.

Early goal-directed therapy: Early goal-directed therapy (EGDT) is a resuscitation strategy aimed at achieving specific hemodynamic targets (such as central venous pressure, mean arterial pressure, urine output, and central venous oxygen saturation) within the first 6 hours of sepsis recognition. While EGDT was previously advocated as a standard of care for sepsis management, recent studies have suggested that individualized hemodynamic management based on clinical assessment and dynamic parameters may be equally effective in improving outcomes.

Continued monitoring and reassessment: Patients with sepsis require close monitoring and frequent reassessment of their clinical status, response to treatment, and resolution of infection. Serial physical examinations, vital sign measurements, laboratory tests, and imaging studies may be performed to evaluate for signs of clinical improvement, persistent infection, or complications. Ongoing communication and collaboration among members of the healthcare team are essential for optimizing patient care and ensuring timely intervention.

Overall, the management of sepsis requires a coordinated and multidisciplinary approach involving healthcare providers from various specialties, including emergency medicine, critical care, infectious diseases, surgery, and nursing.
